Voluntary Life Insurance

Insurance provided by Mutual of Omaha


Full-time employees eligible 1st of the month following start date.


Group Life Insurance:

  • Paid by DeKalb CUSD #428 for full-time employees per the terms of the union or employee contract. 
  • Employee needs to complete All Insurance Enrollment form to designate beneficiary.
  • All Insurance Enrollment Form can be found in the Group Life Insurance folder below.
  • Long-Term Disability Insurance: Optional voluntary benefit available to all full-time employees subject to application approval. Cost to employee is reasonable. Can only be modified during open enrollment or upon a qualifying event.
  • Monthly benefit per schedule, depending on disability.
  • Benefit begins per schedule, depending on disability.

  • Click on Long Term Disability Insurance folder
  • Click on LTD Calc Worksheet; Download Worksheet to desktop
  • Click on Excel file. It will open in Exel. Click on Enable Editing. 
  • Enter information in yellow as it pertains to you. This information will be entered on your enrollment form.

 

Voluntary Life Insurance:

  • Optional voluntary benefit available to all full-time employees, employee's spouse and dependent children up to the age of 26. Cost to employee is reasonable. Can only be modified during open enrollment or upon a qualifying event.
  • Upon initial hire, employee can obtain up to $240,000 guaranteed issue up to a maximum of $500,000 upon Evidence of Insurability.
  • Upon initial hire, employee's spouse can obtain up to $30,000 guaranteed issue up to a maximum of $100,000 upon Evidence of Insurability.
  • Upon initial hire, employee's dependent children (up to the age of 26) can obtain up to $10,000 guaranteed issue.
